The tribute walk is a way that we honor, celebrate and remember patients during The Blast. This year, Cook Children’s is gifting tribute signs at no cost to you! The signs include the child’s photo, name and diagnosis, and will indicate angel, fighter or survivor. After March 6, families may pick up their signs. A parent or guardian must complete photo and HIPAA consents online. You may choose a free tribute sign when you register. Your confirmation email will include a survey so that you can fill in your sign information.

PLEASE NOTE: The deadline has been extended! All information for a complimentary, pre-printed tribute sign is due by Friday, February 12. The step-by-step process for completing your sign can be found here.

We will let you know when your tribute sign arrives. Families may also collect their signs during packet pick-up. Cook Children’s is not responsible for tribute signs that are not claimed within a reasonable time frame after March 6.
